PETITION FOR PROBATE OF WILL IN THE                                       COURT OF

          COMES NOW, the undersigned being individual, a resident of                                                                            (address),                         (state), and for this Petition for Probate of Will (or Application) states the following:

          1.  The undersigned seeks to be appointed as the Executor for the Decedent’s affairs as above-captioned.

          2.  The Decedent                                                                       (name) of                                                                                                        (address) died on the             day of                                   ,             and at the time of death was domiciled in the state of                                    at the aforementioned address.

          3.  During the course of the Decedent’s lifetime, the Decedent duly executed a document known as the Decedent’s Last Will and Testament, which was dated the               day of                                   ,           and as evidenced by the attachment (the Last Will and Testament should be attached).

          4.  The undersigned is desirous of serving as Executor for the Decedent and as evidenced by the attached Will, the undersigned was listed to serve as Executor for the Decedent in this matter.

          5.  It is estimated that the value of the Decedent’s estate is $                   ; therefore, if a bond is required to be posted, the bond should be based upon the aforementioned dollar amount as the noted estimated value of the Decedent’s estate.

          7.  The undersigned has contacted legal counsel for assistance in this matter as follows:

          WHEREFORE, the undersigned respectfully requests that this Petition (Application) be granted relative to the acceptance of the attached Will for Probate and the appointment of the undersigned as the Executor pursuant to the attached Will and that bond, if necessary, be posted pursuant to the amount estimated to be the value of the estate as noted above.
