	Recently, it was brought to my attention that an adverse credit report
was provided in regard to the undersigned.  This report must be erroneous due
to the fact that the undersigned does not have, nor has the undersigned had,
an account with the party who reported adversely relative to the credit standing
of the undersigned.  Kindly verify your records since it appears as though a
name confusion has arisen, and this mistake should be immediately corrected.
Apparently, there is another individual with the same name as, or a similar name
to, the undersigned; no doubt that has led to the confusion.  It does seem
appropriate that an apology would be in order.  However, I am mostly concerned
with the correction of this problem and would appreciate notification to
appropriate agencies who have received this information in error about me.
