  1. Ordering that the custody paragraph of the Judgment and Decree be amended to allow Petitioner to remove the children from the state.

  2. Ordering that the visitation paragraph of the Judgment and Decree be amended to revise Respondent's visitation rights.

  3. For such other and further relief as the Court deems equitable under the circumstances.

  4. This Motion will be based upon Petitioner's Affidavit, the entire court file, and arguments of counsel at the hearing.
